Unilateral leucokoria, combined cataract congenital, and persistent fetal vasculature presentation in a baby

Abstract Details

Purpose:

To present a case of leukocoria in a baby with unilateral leukocoria, thus difficulty to distinguish between Persistant Fetal Vasculture (PFV) or Combine Persistent Fetal Vasculature and Congenital Cataract, and discuss further management for this patient

Setting:

Department of Ophthalmology, Faculty of Medicine Airlangga University, Dr. Soetomo Hospital, Surabaya, Indonesia

Methods:

A 4 months old boy presented with unilateral leukocoria affecting his left eye since birth. During examination revealed visual acuity 6/480 (lea grating), opaque lens without red reflex and normal B-scan ultrasonography. His unusual laboratory finding was positive IgG Rubella, IgM anti CMV, and IgG anti CMV. No surgical intervention was done and patient was kept on regular follow.

Results:

During Examination Under Anesthesia (EUA), intraocular pressure was 14.6 mmHg, cornea vertical diameter was 10 mm and horizontal diameter was 10.5mm, 2 o-clock posterior synechiae, opaque lens with blood vessels. The affected eye show steady progress on visual acuity within 3 months follow up

Conclusions:

Combined PFV and Congenital cataract in one unilateral eye is uncommon condition. This case highlights the need of diagnose step to distinguish between PFV, Cataract congenital or combined condition
