Pupil dilators in surgery of complicated cataracts

Abstract Details
Purpose:
To evaluate the efficacy and safety of pupils expanders in outpatient surgery of complicated cataracts in patients with narrow rigid pupils.

Department of eye diseases of the "Samara state medical University",
Ophthalmic clinic "Eye Surgery", Samara, Russia
Methods:
Outpatient surgery of 27 patients with complicated cataracts and narrow rigid pupil, which made phacoemulsification with IOL implantation of the company "ALCON". During surgery we used two mechanical dilators to improve pupil – "Malyugin ring" production ETP "Eye microsurgery" and ZAO "NPO "Ice". The average age of patients was 63.8±7.8 years; women – 21 (78%), men – 6 (22%). Average best-corrected visual acuity (BCVA was increased) was 0.12±0,11 (0,01-0,3); IOP of 19.9 ± 3.8 mm Hg. art.
Results:
There were no operational complications. In the postoperative period in 5 (18%) patients experienced elevation of IOP, which was offset by a medication; in 4 (15%) of patients had corneal edema, which itself was stopped a week after surgery. As a result of treatment, uncorrected visual acuity was 0.52±0.02. The average spherical clinical refraction was 0,41±0,38 diopters. Refraction was achieved in 91% of patients. 3 patients with dental implants "AcrySof IQ Toric" the magnitude of the astigmatism according to autorefractometry was reduced to 0,53±0,04 diopters. Loss of cells of the corneal posterior epithelium was 4.9±1.9%.
Conclusions:
Both models of mechanical pupils expanders production ETP "Eye microsurgery" and ZAO "NPO "Ice" - provide sufficient midriaz strictly in the plane of the pupil, atraumatic, maintain a stable position throughout the operation, are introduced and removed through the main tunnel incision, ensure the safety of the diaphragm pupil function.
In our practice of outpatient surgery of complicated cataracts, we often (77.7%) used the "Malyugin ring" produced by the ETP "Eye microsurgery", which provided a slightly larger pupil diameter due to the peculiarities of its design.
