Comparison of two versions of combined ultrasound settings for hard nucleus removal with the help of femtosecond laser

Abstract Details

Purpose:

The purpose of this study was to assess the main ultrasound and hydrodynamics parameters after hard nucleus removal using two types of combined (longitudinal + torsional) ultrasound (US) settings.

Setting:

S. Fyodorov Eye Microsurgery Federal State Institution. Saint-Petersburg branch.

Methods:

The study included 20 patients with hard cataracts (NC6+). After femtolaser pretreatment each nucleus was divided into 8 identical fragments. 4 fragments were aspirated using ultrasound settings №1 with the following time interval: longitudinal – 0-10ms, torsional – 16ms, pause – 16-6ms. The other 4 fragments were aspirated using settings №2: longitudinal US – 30ms, torsional US – 30-100ms, pause – 20-50ms. After removal of each piece the following parameters were recorded: Cumulative Dissipated Energy (CDE), aspiration time and fluid consumption. Both settings have been tested by means of the virtual simulator program prior to the surgery.

Results:

After virtual testing settings №1 showed lower CDE level. After real surgical condition testing settings №1 were the best in all analyzed parameters (p<.05).

Conclusions:

CDE, aspiration time and fluid consumption during hard lens material removal is lower when you use settings with increasing longitudinal US time and fixed torsional US time with the foot pedal in the third position.
