Influence of the irrigation-port location of the phaco sleeve on ophthalmic viscosurgical device retention during phacoemulsification

Abstract Details
Purpose:
During phacoemulsification, the corneal endothelial protection depends on the retention of the ophthalmic viscosurgical device (OVD) in the anterior chamber, which can be affected by the fluid flow. We observed the alteration of the retention of the OVD by the irrigation port direction of the sleeve using a slitlamp microscope.

Ophthalmology, Nippon Medical School, Tokyo, Japan
Methods:
Phacoemulsification was simulated in porcine eyes using 2 different types of phaco sleeves; one was the conventional sleeve with 2 ports directed to horizontally opposite side (Conventional group), and the other was the new sleeve with 2 ports faced to the same direction. With the new sleeve, the ports were directed to downside during phacoemulsification (New group). Using the fluorescein stained viscoadaptive OVD, movement of the OVD in the anterior chamber was observed by a slitlamp microscope.
Results:
In the conventional group, the irrigation flowed between the corneal endothelium and the OVD, while such flow was not observed in the new group in which the irrigation ports were directed downside during the phacoemulsification. Consequently, the retention of the OVD in the corneal endothelial side of the anterior chamber was better in the new group than the conventional group.
Conclusions:
The new sleeve with ports which faced the same direction was useful for the control of irrigation flow direction, and thus, the retention of the OVD during phacoemulsification.
