Axial length as a factor associated with visual outcome after uncomplicated phacoemulsification cataract surgery

Abstract Details

Purpose:

To compare the axial length (AL) of the eye before and after uncomplicated phacoemulsification cataract surgery measured with applanation ultrasound and to analyse correlation with the best corrected visual acuity (BCVA) and main intraoperative phaco parameters (CDE, phaco time, estimated fluid used)

Setting:

Prospective cohort study included 30 consecutive patients scheduled for uncomplicated phacoemulsification cataract surgery at the Department of Ophthalmology, University Hospital Centre Osijek,School of Medicine, University J.J. Strossmayer Osijek, from October 2017 to January 2018.

Methods:

Only patients with cataract and no other ophthalmic pathology or diseases were enrolled. AL was measured using applanation ultrasound (Nidek US-4000 ultrasonic biometric device) before and one month after surgical intervention. Intraoperative measurements (Cummulative dissipated energy (CDE),ultrasound time, and fluid volume) were also registered. BCVA was registered before and one month after surgery and was correlated with axial length values. Mean preoperative AL was 22,25 (range 21,87-23,10 mm) and postoperative 22,29 (range 21,82-23,08mm).There was not statistically significant difference in AL before and after surgery (p=0,25,Wilxocon test).

Results:

7 of 30 patients had AL change but difference in their AL was not significantly different comparing to 23 patients who did not have it (p=0,74 Wilxocon test).BCVA before and after surgery was without significant difference between two groups of patients considering whether AL changed or not (p=0,38,p=0,86 Wilcoxon test; Mann Whitney U test).Median CDE was 11,93(8,84-14,67 p=0,02) and it was significantly lower in patients with AL changes.The phaco time during surgery was 56± 6 seconds (p=0.57),the estimated fluid used was 50,5 (30,25-66,50)(p=0.84MannWhitney U test)ccm3, and without statistically significant relationship between these parameters and the difference in the AL measurement.

Conclusions:

The difference in AL measurement before and after uncomplicated phacoemulsification cataract surgery was not statistically significant and had not statistically effect on BCVA. CDE was significantly lower in patients with AL changes. However, fluid volume used and phaco time had no effect on AL change, neither on BCVA. Applanation ultrasound A scan biometry proved to be accurate AL measurement method which is important and safe in predicting ideal postoperative refractive result.
