Helvacioglu coefficient optimising factor for torsional phacoemulsification

Abstract Details

Purpose:

To compare the safety and efficacy of Ozil Intelligent Phaco (IP) torsional microcoaxial phacoemulsification surgeries performed by 45º aperture angled balanced and 22 degree bent tips by using Helvacioglu coefticient optimising factor (HCOF).

Setting:

Veni-Vidi Eye Hospital, Istanbul

Methods:

Eighty eyes were assigned to 2.2 mm microcoaxial phacoemulsification using Ozil IP torsional mode with balanced (Group 1) or 22º bent (Group 2) tips. The primary outcome measures were US time (UST), cumulative dissipated energy (CDE), longitudinal and torsional ultrasound amplitudes, mean operation time, mean volume of BSS used and mean central corneal thickness (CCT) changes. CDE was also recalculated by using a new optimising factor.

Results:

Mean UST, CDE, CDE optimised by HCOF, longitudinal and torsional ultrasound amplitudes were 59.25 sec. ± 42.13 sec., 5.22 ± 4.90, 7.04 ± 6.61, 0.18% ± 0.11%, and 18.34% ± 8.01% in Group 1 and 58.80 sec. ± 32.29 sec., 8.01 ± 6.00, 7.20± 5.40, 0.40% ±0.31% and 27.99% ± 12.89% in Group 2 respectively. Mean CDE and US amplitudes were found to be significantly low in Group 1 (p=0.018, p<0.001, p<0.001). Mean CCT changes were 48.02 µm ± 30.72 µm in Group 1and 54.21 µm ± 36.06 µm in Group 2 respectively (p=0.361).

Conclusions:

The Ozil IP torsional phacoemulsification performed by 45º aperture angled balanced tips provided more effective lens removal than 45º aperture angled 22 degree bent tips with currently available energy measurements. When HCOF optimised CDE was considered, similar efficiency was observed in both groups.
