The dynamics of tear production, preoperatively and at different terms of postoperative period, of phacoemulsification in patients with senile cataract

Abstract Details

Purpose:

The problem of prophylaxis of “dry eye” syndrome (DES), which occurs after different ophthalmic operations, is very important for practical ophthalmology. The frequency of DES development which occurs after surgical interventions on the anterior segment of the eye varies from 3.6 to 16 % (according to different data). The purpose of our work was to study the dynamics of tear production in patients with senile cataract before and at different terms after phacoemulsification (PE).

Setting:

The work was performed in the State Institution Eye Microsurgery Complex named after academician S.N. Fyodorov, Khabarovsk, Russia.

Methods:

136 patients (136 eyes) with senile cataract aged from 62 to 83 (mean 69.3±6.4) years. There were 64 men and 72 women. All patients underwent PE using "Infiniti» (Alcon) phacomachine with implantation of different models of acrylic IOLs. We have performed Schirmer's test in all patients before operation, on the first, 7th, 14th and 30th postop day. Patients with severe symptoms of DES, infectious diseases of anterior segment of eye and with glaucoma were not included into investigation.

Results:

All patients were divided into 4 groups. The first group consisted of 32 eyes (Schirmer's test 15 mm and more); second group – 48 eyes (10-15 mm); third group – 40 eyes (5-10 mm) and fourth group – 16 eyes (less than 5 mm). On the 7th day tear production was lower than its initial level. Gradual restoration of tear production occurred to the 30th postoperative day. The indices of Schirmer's test were lower than initial in 4 eyes of the first group, 10 eyes of the second group, 31 eyes in third and 10 eyes in the fourth group.

Conclusions:

The performed study of dynamics of tear production in patients with senile cataract before and in different terms after PE showed, that production of tear fluid recovers up to initial level only in 65,9% of eyes during first postoperative month. Despite of initial level of tear production, its universal dynamics with primary increase, following decrease and return to its original values was found in all eyes. High risk of DES development at PE had place in 56 (41.1%) eyes due to moderate and severe suppression of tear production.
