Prevention of intraoperative pupillary miosis in extracapsular cataract extraction with nepafenac 0.1% topical drops

Abstract Details

Purpose:

To determine the role of topical 0.1% nepafenac drops in preventing intraoperative pupillary miosis in extracapsular cataract extraction

Setting:

Khyber Teaching Hospital, Peshawar, Pakistan

Methods:

This single center, prospective, randomized clinical study included 58 patients planned for extracapsular cataract extraction. Patients were randomized to nepafenac 0.1% group (n=28) or to no treatment control group (n=30). Nepafenac 0.1% drops were administered 3 times every 10 minutes, 30 minutes before surgery. Pupillary diameter was measured with caliper just before the procedure and at the conclusion of procedure. Primary outcome was to measure change in pupillary diameter at the conclusion of surgery between the two groups. Secondary outcome was to measure the relative risk of > 1.0mm miosis in the two groups.

Results:

At the conclusion of surgery the difference of pupillary diameter in nepafenac 0.1% group (7.41 ± 0.67 mm) and the control group (6.85 ± 0.82 mm) was statistically significant (P=0.006). All patients had more than 6mm pupil dilatation before surgery. The relative risk of ≥ 1 mm miosis is 2.22 times more in the control group than the nepafenac group which was statistically significant (P=0.008).

Conclusions:

Use of topical nepafenac 0.1% is beneficial in preventing intraoperative pupillary miosis in brown irides undergoing extracapsular cataract extraction.
