A case of high degree refractive error after intraocular lens scleral fixation

Abstract Details
Purpose:
Intraocular lens (IOL) scleral fixation technique is useful technique and it is becoming popular for cataract surgeons. However, we experienced a case of visual function improvement because of high degree refractive error.
Setting:
Department of Ophthalmology, Dokkyo Medical University, Tochigi, Japan.
Methods:
An 86-yaer-old man visited our hospital complaining of visual disturbance in left eye. The patient had primary cataract surgery without IOL implantation because of capsule dislocation at private clinic on July 2015. He had scleral fixation using 3 pieces IOL (X-70, Santen) at same clinic on September 2015. However, his corrected distance visual acuity (CDVA) is 20/50 with -4.5D myopia and -9D astigmatism. The IOL was highly inclined. We extracted the IOL and implanted new IOL (X-70) using double needle technique (Yamane S: Ophthalmology, 2017).
Results:
The CDVA was 20/40 after exchange the IOL, but the refractive error was decreased to -0.75D and astigmatism decreased to 0D. The subjective visual function was improved. The highly deformed haptics of extracted IOL was recognized using microscope after the IOL exchange.
Conclusions:
The haptics deformation occurs to high myopia and astigmatism. Unaccustomed or rough operations may increase refractive error to the scleral fixation technique.
