Macular thickness changes and visual outcomes in diabetic patients following uncomplicated phacoemulsification cataract surgery

Abstract Details

Purpose:

To analyse the effect of uncomplicated phacoemulsification cataract surgery on macular thickness changes and visual outcome in the early postoperative period in diabetic patients using optical coherence tomography (SD-OCT). This prospective study included 55 patients divided in two groups.The first group consisted of 25 diabetic patients and the second was control, consisted of 30 patients with no history of diabetes nor any other ophthalmic disease.Exclusion criteria were previous eye surgery,uveitis,non diabetic retinal disease,earlier macular edema or thickening of any ethiology,laser photocoagulation within 3 months before cataract surgery,choroidal disorders, proliferative diabetic retinopathy,mature cataract,pupil seclusion or progressive corneal degenerations.

Setting:

Uncomplicated phacoemulsification cataract surgery were performed by the same surgeon at the Department of Ophthalmology,University Hospital Centre Osijek,School of Medicine, University Josip Juraj Strossmayer Osijek, in the period from October 2017 to January 2018.Patients were followed 1 month after cataract surgery.

Methods:

The best corrected visual acuity (BCVA)using Snellen chart was measured,Slit lamp grading of lens opacification according to the Lens Opacities Classification System III (LOCS III)was done and all underwent SD-OCT macular thickness measuring of eye planned for surgery at each control.The first control was performed one week and the second,one month after surgery.HbA1c blood sample level was measured preoperatively in diabetic patients.Cummulative dissipated energy (CDE) and estimated fluid used (ESF) (ccm³)were measured as the most important parameters during surgical procedure,which may be related to macular thickness changes in the early postoperative period.

Results:

There was statistically significant difference in macular thickness before surgery and the second control (p=0.0045137)and between the first and the second control in diabetic(p=0.0069024)as well in the control group before surgery and the second control (p=5.2657×10(-4),and between the first and the second control(p=0.01285).There was no statistically significant difference in macular thickness between diabetic and control group preoperatively(p=0,618),on the first(p=0.6848) and the second control (p=1).Correlation between CDE and macular thickness in both groups was not statistically significant at the first(p=0.25858, p=0.40211),nor at the second control(p=0.38402,p=0.8416) as well as correlation of ESFand macular thickness on the first(p=0.624,p=0.90735)and on the second control.

Conclusions:

The correlation between HbA1c level and macular thickness was not statistically significant on the first (p=0.48405) and the second control (p=0.74875). Uncomplicated phacoemulsification cataract surgery causes macular thickness changes in early postoperative period in both groups of patients. . Although phacoemulsification cataract surgery significantly reduced the proportion of early postoperative complications, this study has been shown to have an effect on macular thickness changes in the early postoperative period, which is proved using SD-OCT but with no evident clinically significant macular edema. In both groups of patients, BCVA was improved.
