Phacoemulsification with intraocular lens implantation in patients with Behcet’s disease: complications and visual outcomes

Abstract Details

Purpose:

To evaluate the visual outcomes and complications of phacoemulsification in patients with Behcet s disease.

Setting:

Gozakademi Eye Hospital,Department of Ophthalmology,Denizli,Turkey

Methods:

Chart reviewed of 90 eyes that underwent phacoemulsification with hydrophobic foldable IOL implantation from 2007 to 2017 for the treatment of visually significant cataract in Behcet’s disease .The mean age of the patients was 32.9 years (range 24 to 54 years). Mean postoperative follow-up was 22.6months (range 5 to 66 months). Visual Acuity (VA), anterior segment inflammation, iris and capsular features, IOL position , intraocular pressure, complications within surgery were measured and reported.All eyes were in remission regarding active uveitis with systemic immunsupresive treatment for at least 6 months before the surgery.

Results:

All surgeries were successfully completed with the implantation of the IOL in the capsular bag.All eyes demonstrated an improvement in BCVA. Mean BCVA preoperatively was 0,95±0.18 logMAR and postoperatively at the final visit was 0.32±0.2 logMAR(p<0.05). Posterior capsule opacification(PCO) requiring neodymium:YAG capsulotomy occurred in 43 eyes; posterior synechias in 14 eyes; cystoid macular edema(CME) in 17 eyes; recurrent uveitis in 17eyes; epiretinal membrane formation, glaucoma, and iris bombe in 5 eyes each. Two eyes required additional IOL exchange surgeries due to severe opacification.

Conclusions:

The main complications affecting visual outcomes were PCO, CME and recurrent uveitis.With appropriate preoperative and postoperative suppression of inflammation, phacoemulsification and intraocular lens implantation are safe procedures leading to visual improvement in most patients with Behçet's disease induced cataracts.
