Abstract Details

Purpose:

To determine the possibility of intraoperative floppy iris syndrome (IFIS) preoperatively by consulting anatomical features of iris via slit-lamp biomicroscopy.

Setting:

Prospective, randomized trial

Methods:

This study includes 152 male patients who undergone uneventful cataract surgery in Erzurum Region Training and Research Hospital. All participants were examined with detailed ophthalmologic examination preoperatively and recorded slit-lamp biomicroscopic findings as dark pigmented iris, atrophic iris, marked stromal fibers, peripupillary atrophic iris, fuchs criptas, light-coloured iris, pigmented spots on iris, iris pigments on anterior lens surface and iris nevus. Participants were divided into two group as with/without IFIS. The diagnosis of IFIS was defined as the presence of iris billowing, progressive miosis or iris prolapsus.

Results:

77 IFIS patient and 75 control participants were compared with preoperative biomicroscopic findings. the prevelance of iris anatomical features were %20.8-14.3 with dark pigmented iris, %11.7-11.4 with atrophic iris, %34.3-28.6 with marked stromal fibers, %5.7-3.9 with peripupillary atrophic iris, %28.6-20.8 with fuchs criptas, %9.1-2.9 with light-coloured iris, %3.9-2.9 with pigmented spots on iris, %1.3-2.9 with iris pigments on anterior lens surface and %5.7-1.3 iris nevus respectively in IFIS and control group. there was statistically significant difference in Fuchs criptas and light-coloured iris. (p=0.014; p=0.008).

Conclusions:

Intraoperative floppy iris syndrome is related with significant complications. Therefore the prediction of this syndrome is important to prevent complications and take precautions preoperatively. We hypotised that biomicroscopic iris features are related with the presence of IFIS and our results showed that %71.4 of IFIS patients have at least one of these findings and %80 of non-IFIS patients have any iris anatomical iris feature. On the whole entire, our hypothesis was not supported by results except for two parameters. Large scaled studies needed for certain results
