Abstract Details

Purpose:

To analyze the long-term results of the suggested method of transciliary suture fixation of the three-piece IOL and alternative methods of fixation in eyes with complete absence of capsular support.

Setting:

Federal State Autonomous Institution "S.N. Fedorov National Medical Research Center "MNTK "Eye Microsurgery" of the Ministry of Health of the Russia, Volgograd.

Methods:

216 patients (216 eyes) with aphakia with total absence of capsular support were examined. In the first group (65 eyes), a three- piece IOL Secura-sSAY (Human Optics, Germany) was implanted with a original suture transciliary fixation. In the second group (57 eyes), IOLs of PMMA were implanted with suture fixation in the ciliary sulcus using standard technology. In the third group (59 eyes), an anterior chamber IOL was implanted and in the fourth group (35 eyes) IOL was implanted in the posterior chamber with a suture fixation to the iris.

Results:

Early postoperative period was uneventful in 51 eyes (78.5%) in the first group, in 44 eyes (77.2%) in the second group, in 38 eyes (64.4%) in the third group, in 23 eyes (65.7%) in the fourth group. Mean values of BCVA 1 year after surgery were 0.61 ± 0.21 in the first group, 0.53 ± 0.2 in the second group, 0.4 ± 0.2 in the third group, 0.5 ± 0.5 in the fourth group. The difference between the first group and the rest is statistically significant (p < 0.05).

Conclusions:

The proposed method of suture fixation of the three-piece IOL in the ciliary sulcus with the complete absence of the lens capsule allowed to reduce the surgical trauma, to obtain high visual functions, and stable position of the IOL throughout the observation period.
