Abstract Details

Purpose:

To study anterior segment changes after implantation of Keraring 355˚ arc length in central advanced keratoconus.

Setting:

Menoufia University Hospital and Tiba Eye Center. Sheibin El Kom Faculty of Medicine, Menoufia University, Egypt.

Methods:

Thirty eyes of 22 patients with grade 3 central keratoconus and absence of corneal scaring underwent femtolaser assisted (Visumax) implantation of intracorneal ring segment of 355° arc length intrastromal corneal ring segment (ICRS) with segment thickness of 300 µm. Comprehensive ocular evaluation was performed preoperatively as well as postoperatively regarding unaided visual acuity, BSCVA, K1, K2, K max, corneal volume, Anterior chamber depth (ACD), corneal elevations and actual segment position 6 months after the procedure.

Results:

Best spectacle corrected visual acuity (BSCVA) in decimal notation improved significantly from 0.22±0.17 to 0.49±0.22 (P≤0.001). Statistically significant changes were found regarding the measured parameters with marked reconstruction of the cornea as well as anterior chamber. However; actual depth of ring segment was 61.7±5.9% which was shallower than intended depth of 80% of corneal thickness.

Conclusions:

Those findings may help to explain the marked improvement of visual parameters obtained with such type of rings in advanced keratoconus; moreover those types of rings may be a safe, effective and minimally invasive way to avoid corneal grafting in advanced keratoconus with clear cornea.
