Comparison of maximum stretch force and the morphology of anterior lens capsulotomy across three different methods

Abstract Details

Purpose:

To compare the maximum stretch force and morphology of the anterior lens capsulotomy edge between Continuous Curvilinear Capsulorrhexis (CCC), Femtosecond Laser Capsulotomy (FLC), and Precision Pulse Capsulotomy(PPC).

Setting:

The study was conducted at Chukyo Eye Clinic, Nagoya, Japan

Methods:

Fresh porcine eyes were used in this study. CCC, FLC, and PPC were created using forceps (10 eyes), LenSx® (Alcon; 12 eyes) and ZEPTOTM (Mynosys; 10 eyes), respectively. After the capsulotomy and removal of lens material, the empty lens capsule was removed from the eyeball. Each capsulotomy edge was pulled by two L-shaped jigs at constant speed until an edge tear appeared. The morphology of the capsulotomy and microstructure of the capsulotomy edges were studied using ImageJ and scanning electron microscopy (SEM), respectively.

Results:

The maximum stretch forces in the FLC group (108.8±14.7 mN) were significantly less than that in CCC (173.7±44.9 mN) and PPC (147.5±34.7 mN) groups (p = 0.0002). Differences in size were not significant. The circularity was lower in CCC than FLC group (p = 0.003). The eccentricity distance of capsulotomy was less in FLC and PPC than CCC group. The edge of the capsulotomy was smoothest in CCC and least so in FLC group, on SEM observation. The cut edges in PPCs were everted externally with an additional functional edge.

Conclusions:

On comparison of the maximum stretch force in each capsulotomy group, keeping the size identical, FLC was significantly weaker than CCC and PPC groups. The morphology of the cut edge was different among the various methods. The microstructure of the anterior capsulotomy edges may affect the maximum stretch force.
