XEN45 implant for glaucoma: 6 month results of efficacy, safety profile, and postoperative management

Abstract Details

Purpose:

To ascertain the efficacy, safety profile and post operative management of XEN45 implants at 6 months follow -up

Setting:

This study was a retrospective, non randomised interventional case series of patient undergoing XEN45 implant by a single, experienced glaucoma specialist at an NHS tertiary centre. Venue: North Cumbria University Hospital NHS Trust

Methods:

Primary outcome measures were intraocular pressure (IOP) at 1 month, 3 month and 6 month follow up. IOP criteria was chosen to be successful if IOP < 21mmHg or 20% reduction from baseline IOP. 14 eyes completed 6 month follow up with 3 eyes completing 3 months follow up. Post-operative complications were recorded at every visit. Retinal nerve fibre layer thickness and number of topical medications used were also recorded at every visit.

Results:

The mean preoperative IOP was 28.7 ± 4.9 mmHg, which reduced to 19 mmHg ± 11 at 1 month (Two-tailed T-test,p = 0.009). At 3 months, the mean IOP reduced to 16.5mmHg ± 4.5 mmHg (p <0.001). At 6 months, the mean IOP was 15.05 mmHg ± 2.68 mmHg (p-value < 0.001;95% CI: 10.80 – 15.80).Success rate achieved in 73% of the patients. Bleb needling was required in 65% of eyes. 1 (5.5%) implant tube was noticed to be broken at 1 month. Re-implantation was performed. No cases of hypotony was found. The lowest IOP obtained was 9 mmHg post-operatively.

Conclusions:

The XEN45 Implant has proved to be an effective means to lower Intra ocular pressure at 6 months. The IOP lowering effect obtained at 1 month is maintained even at 6 months however, there is a high rate of bleb complications requiring bleb needling. We recommend XEN45 implant in carefully chosen candidates with close follow up to avoid acute complications.
