Biometric and refractive changes after Ex-PRESS shunt implantation

Abstract Details

Purpose:

To assess and reveal mechanism of biometric and refractive changes after Ex-Press shunt implantation.

Setting:

Saint-Petersburg Multifield Hospital 2

Methods:

In total, 19 eyes of 19 glaucoma patients (8 male and 11 female) undergoing Ex-PRESS shunt implantation. Observation time: from April 2017 to October 2017 (6 months). Refractometry, keratometry, axial length measurement, IOP (Topcon KR – 8800, IOL-Master 500 Carl Zeiss, iCare) was performed for each patient before shunt implantation, in the 1st postsurgical day, in 1 month and in 6 months after surgery.

Results:

There was mild hyperopic refractive shift from -1,64 ± 2,34 D baseline level to -1,30 ± 2,21 D 1 month after surgery. However, refraction came back to presurgical data -1,53 ± 2,28 D . There was no changes in keratometry values. It was 44,77 ± 0,96 D before surgery and 44,69 ± 0,97 D 6 months after shunt implantation. There was moderate positive correlation (R = 0,605) between IOP and Axial length (AL), expressed in AL shortening due to IOP lowering (from 23,46 ± 0,82 mm before Ex-PRESS implantation to 23,39 ± 0,80 mm 6 months after surgery.

Conclusions:

Data analysis revealed mild hyperopic shift on 0,11 D 6 months after Ex-PRESS shunt implantation. Supposed mechanism of this changes is AL shortening from 23,46 ± 0,82 mm to 23,39 ± 0,80 mm due to IOP lowering. There was insignificant fluctuation of corneal refraction (CR) from 44,77 ± 0,96 D to 44,69 ± 0,97 D and ACD values from 2,89 ± 0,28 mm to 2,89 ± 0,22 mm in postoperative period. Thus, role of CR and ACD changes in refractive shifts after Ex-PRESS shunt implantation is minor.
