Abstract Details

Purpose:

To compare topical 2% rebamipide drops with a combination of polyethylene glycol and propylene glycol eye drops for the management of lid wiper epitheliopathy in Sjogren’s syndrome in mild to moderate keracoconjunctivitis sicca [KCS]

Setting:

An open label randomized control trial at Advanced Eye Center, PGIMER, Chandigarh, India

Methods:

In this prospective, open label study, 40 diagnosed patients of Sjogren’s syndrome meeting the criteria of mild to moderate keartoconjunctivitis sicca were randomized [into two groups of 20 each] to receive either 2% rebamipide drops qid (treatment group )or 0.4% polyethylene glycol 400 and 0.3% propylene glycol 400 drops qid[ control group] for a period of 4 weeks. Ocular surface disease index [OSDI], tear film break up time [TBUT], ocular surface staining, and Lid wiper epitheliopathy (LWE) were evaluated at baseline and at the end of 4 weeks and were compared between the two groups.

Results:

Baseline parameters were same in both groups. At the end of study OSDI scores decreased significantly in treatment group (18.7 ± 5) as compared to control group (15.1±5.2) (p value 0.03). TBUT also showed significant increase in treatment group (1.7±1.2) as compared to control group (0.8±0.72) (p value 0.02). Ocular surface staining improved comparably in both the groups with p value>0.05. Significant improvement in Lid wiper staining (55% of patients with grade 0 staining) was seen in treatment group than in the control group (20%) at 4 weeks (p value 0.02).

Conclusions:

Topical 2% rebamipide is more effective than polyethylene glycol and propylene glycol combination eye drops in improving the subjective symptoms of dry eye as well as objective parameters including TBUT and lid wiper staining in mild and moderate cases of KCS in Sjögren’s Syndrome.
