Wavefront-optimised vs wavefront-guided LASIK using a high resolution aberrometer: one year results

Abstract Details

Purpose:

To prospectively compare outcomes between wavefront-guided LASIK and wavefront-optimized LASIK in the treatment of myopia and compound myopic astigmatism. Outcome measures include high contrast snellen acuity, low contrast snellen acuity (5 and 25%), safety, predictability, efficacy and higher order aberration analysis.

Stanford University School of Medicine

Methods:

200 eyes of 100 consecutive patients were treated with wavefront-guided and wavefront-optimized LASIK. Wavefront measurements were performed using the high-resolution J & J Vision iDesign aberrometer. One eye was treated with wavefront-guided LASIK using the J & J Vision Visx S4 excimer laser and the fellow eye was treated with wavefront-optimized LASIK using the Alcon Allegretto Wave Eye-Q excimer laser Eyes were randomized according to ocular dominance. The mean pre-operative spherical equivalent refraction was -4.55 +/- 1.98 and -4.55 +/-2.01 in the WFG group and WFO group respectively.

Results:

Mean spherical equivalent refraction was -0.10 +/- 0.36 in the WFG group and -0.24 +/- 0.28 in the WFO group at twelve months. Mean cylinder was reduced to +0.22 +/- 0.20 in the WFG group and +0.15 +/- 0.19 in the WFO group at 12 months. There were statistically significant differences in 5% contrast visual acuity (p=0.002) and 25% contrast visual acuity (p=0.05) with greater gains in the WFG group compared to the WFO group. There were statistically significant greater gains in two or more lines of corrected distance visual acuity in the WFG compared to the WFO (p=0.003).

Conclusions:

Wavefront-guided and wavefront-optimized LASIK both have excellent clinical outcomes. However, wavefront-guided LASIK yielded better UDVA, gains in CDVA, and gains in 5% and 25% contrast visual acuity compared to wavefront-optimized LASIK.
