Gas-bubble escape sign: new intraoperative sign to predict optical quality after SMILE

Abstract Details

Purpose:

To describe a new intraoperative sign called ‘Gas bubble escape’ sign to predict ease of dissection and immediate post-operative quality of vision after SMILE.

Setting:

Nethradhama Superspeciality Eye Hospital, Bangalore

Methods:

This sign refers to the observation of escape of gas bubbles upon opening of the access incision in SMILE. A prospective study was conducted in which 25 eyes(25 patients) showing the gas bubble sign intraoperatively were compared with 25 eyes(25 patients) which did not show this sign during SMILE surgery for myopia correction. Single surgeon performed all surgeries using the same technique and energy parameters( energy=150 nj , 4.5 µ spot spacing).On day 1 and 15, uncorrected distance visual acuity(UDVA) and Optical Scatter Index(OSI) was compared between the two groups.

Results:

On day 1, the mean OSI score was significantly lower (0.4 ±0.15) in the eyes which showed the gas bubble sign, when compared to the eyes which did not show this sign( day 1, mean OSI=0.6±0.2)( p=0.02). The OSI , however was comparable between both groups at 15 days( p=0.70). The mean UDVA also was comparable between the two groups at both day 1 and day 15.

Conclusions:

Observation of Gas Bubble sign may be a useful predictor of immediate visual quality after SMILE suggesting ideal energy and perfect bubble pattern
