Impact of biomaterial change on the pioneer of trifocal diffractive intraocular lens

Abstract Details

Purpose:

Analysis of visual and functional one-month results obtained with the intra ocular lens FineVision HP (Physiol), as the hydrophobic version of the trifocal diffractive MicroF implanted bilaterally in 20 patients.

Setting:

University Hospital - Ophthalmology department, Brest, France

Methods:

Like its predecessor, this IOL combines 2 apodized diffractive optics: one with an addition of +3.5D for near vision and one of +1.75D for intermediate vision, which has demonstrated to offer spectacle independence with correct vision quality maintenance. 3 innovative concepts were implemented: chromatic aberration compensation, anti-glistening hydrophobic acrylic material and wavelet profile on double C haptics for easier unfolding. We conducted a monocentric prospective study of bilateral implantation. Visual acuity at the 3 main distances, defocus curve, spectacle independence, photic phenomena, contrast sensitivity, surgical adverse events and patient satisfaction were assessed at 1 month postoperatively.

Results:

Twenty patients were included in this study: the mean uncorrected binocular visual acuity in far vision (5m), intermediate (60cm) and near vision (33cm) were respectively 0.04+0.06,0.09+0.12 and 0.03+0.04 to 1 month postoperatively in photopic condition. Binocular defocus curves showed the bipolar appearance of distant and near foci with no decrease in the intermediate zone. 90% of patients reported eyeglasses independence and the absence of moderate or significant photic phenomena within 1 month postoperatively. Concerning overall satisfaction, a rate of 95% was found. No intraoperative unfolding problem or IOL decentration were reported.

Conclusions:

Approach of the aberrations measured by Ray tracing does not show any RMS value greater than 0.30 micron. The benefits of hydrophobic material of reducing PCO incidence and absence of glistening require long-term studies. Stability in the bag opens the prospect of adapting a toric pattern. The technological challenge of adapting the design of the trifocal pioneer to a hydrophobic material was met with excellent visual results at all distances while respecting quality of vision. Popularity of hydrophobic material is expected to increase the attractiveness of multifocal in the field of refractive cataracts.
