Corneal tomography changes after femtosecond laser-assisted ICRS for keratoconus

Abstract Details

Purpose:

To assess the changes in tomographic indices 6 months following intracorneal ring segment (ICRS) implantation using Pentacam HR (Oculus, Wetzlar, Germany).

Setting:

Sedaghat Eye Clinic, Mashhad, Iran.

Methods:

Fifty eyes from 50 keratoconic patients who had Keraring (Mediphacos, Belo Horiztone, Brazil) implantation assisted by FEMTO LDV Z6 femtosecond laser (Ziemer Ophthalmic Systems, Port, Switzerland) were included. Pentacam data were analyzed and correlated with distance corrected visual acuity (DCVA), and manifest refraction. Pre and post-operative tomographic indices front maximum keratometry reading (Kmax); Q-value in 8-mm;average pachymetric progression indices (PPI); maximum Ambrosio relational thickness (ARTmax); Belin-Ambrósio enhanced ectasia total deviation index (BAD-D); index of surface variance (ISV), vertical asymmetry (IVA), height asymmetry (IHA) and height decentration (IHD) were compared using the Wilcoxon signed ranks test.

Results:

The pre- and post-operative mean DCVA were 0.32±0.04 and 0.17±0.02 LogMAR, respectively. All indices showed significantly changes (p<0.05) post-operatively compared to the pre-operative status expect average PPI (p=0.20), Q value of the back corneal surface (p=0.08) and IHA (p=0.13). Mean of the indices with significant changes was decreased post-ring implantation. Correlation between change in DCVA and changes of other parameters was not significant statistically. (p>0.05)

Conclusions:

Femtosecond-assisted Keraring implantation is able to regularize the cornea and improve DCVA in keratoconic patients. A considerable flattening effect and reduction in prolateness in the front corneal surface were observed. Scheimpflug tomography effectively detects changes in corneal morphology post ICRS, but the lack of correlations of such changes and improvement in DCVA improvement demonstrate the need for further analysis.
