Reliability of ARTmax and Progmax in diagnosing keratoconus

Abstract Details

Purpose:

To evaluate the sensitivity and specificity of parameters derived from corneal tomography parameters with Pentacam to detect the earliest and most sensitive parameters in diagnosing keratoconus (KCN).

Setting:

Department of Cornea, Ophthalmology Hospital of Anápolis, Anápolis, Goiás, Brazil.

Methods:

This cross-sectional study analyzed data from Pentacam (Oculus Optikgeräte GmbH, Wetzlar, Germany) of 90 patients (180 eyes). Several parameters acquired from Pentacam were analyzed and receiver operating characteristic (ROC) curves were analyzed by evaluating the area under the curve (AUC) to detect the sensitivity and specificity of each parameter.

Results:

There were 20 males (22.2%) and 70 females (77.8%) in the group of 90 patients with a mean age of 36.43 years (SD 14.0 years). The ROC analysis showed that AUC values of the Ambrosio relational thickness maximum (ARTmax) and progress index maximum (ProgMax) were 0.957 and 0.948, respectively. Acoording to the ROC curve, the best cut off point of the ARTmax and Progmax were 320.00 μm and 1.95, respectively. These ARTmax values were associated with sensitivity of 95.7% and specificity of 82.2% and Progmax values of 78.30% sensitivity and 96.20% specificity for KCN (p < 0.05).

Conclusions:

The ARTmax and Progmax are considered highly sentsitive objective parameters in keratoconus diagnosis therefore being especially useful in identifying KCN suspected keratoconus eyes (forme fruste).
