Morphogeometric analysis of corneas treated with cross-linking and correlation with the patient quality of vision

Abstract Details

Purpose:

To describe the application of a new technique based on patient-specific 3D modeling capable to diagnose small variations in the morphology of the cornea due to crosslinking treatment and definition of the correlation with the clinical results obtained

Setting:

VISSUM ophthalmology clinic, Alicante, SPAIN

Methods:

A retrospective observational case series study was carried out including patients without any previous ocular surgery and submitted to the Dresden crosslinking treatment in one eye. For each eye, the optical, visual and tomographic data were analyzed preoperatively and compared with the data obtained 2 months (+/- 1 month) and 9 months (+/- 3 months) after the crosslinking treatment. For each eye a 3D model reconstruction of the cornea was performed using raw tomographic data from Sirius system (CSO, Firenze). 23 morphogeometric variables were defined and correlated with the patient quality of vision.

Results:

A total of 22 eyes from 22 patients aged 14-41 years (23.99 ± 9.63) were included. From the 23 morphogeometric variables defined in the Patient-specific 3D modeling nine have shown a statistically significant (p<0.05) decrease of the corneal volume between the preoperative visit and the 2 months (+/- 1 month) post-operative visit. From these nine variables, five have shown a statistically significant (p<0.05) increase between the 2 months post-operative visit and the 9 months (+/- 3 months) post-operative visit. A very high correlation has been detected between the Posterior apex deviation variable and the RMS Coma (Rho=0,900 (p<0,037)

Conclusions:

Patient-specific geometric models of the cornea can provide accurate quantitative information about the morphogeometric properties of the cornea and describe changes in the corneal anatomy due to crosslinking treatment. This accurate characterization of the cornea enables to get new evaluation criteria to optimize the therapeutic effects of the crosslinking procedures in pathologic corneas.
