Cross-linking for post-LASIK corneal ectasia: flap-on 3-minute and flap-off 30-minute protocol

Abstract Details

Purpose:

To evaluate the outcomes of corneal collagen crosslinking (CXL) for management of corneal ectasia after LASIK.

Setting:

Eye Hospital, Wenzhou Medical University, Wenzhou, 325027, China

Methods:

CXL was performed in 15 eyes of 13 patients either with LASIK flap lift (n=8; 365-nm, 30-mW/cm2, 3 minutes) or with epithelium-on flap-on (n=7 eyes; 365-nm, 3- mW/cm2, 30 minutes) technique. Postoperative change in maximum keratometry (Kmax), spherical equivalent (SE), logMAR uncorrected distance visual acuity (UDVA), and central corneal thickness (CCT) were evaluated.

Results:

There was a significant improvement in SE at 3 (-7.83 vs -6.38; p=0.029) and 12 months (- 7.83 vs -6.13; p=0.036) postoperatively. The Kmax values decreased from 57.2 diopters preoperatively to 55.4 diopters by 3 months and 55 diopters by 12 months postoperatively. The median UDVA changed from 0.82 preoperatively to 0.91 at 3 months postoperatively (p=0.166). The initial improvement in spherical equivalent was more pronounced after flap-off CXL as compared to flap-on CXL (p>0.05). The Kmax flattened more after flap-off CXL whereas there was a slight increase in Kmax after flap-on CXL (p>0.05)

Conclusions:

In our study, collagen crosslinking was successfully used to halt disease progression in post-LASIK keratectasia. We recommend flap-on surgical technique for these cases.
