Abstract Details

Purpose:

A prospective, randomized, double masked, clinical trial was designed to evaluate, visual quality, quality of life and spectacle independence in 152 patients symmetrically and randomly implanted with six multifocal IOLs (Acrysof ReSTOR SV25T0, Tecnis ZKB00, Tecnis ZLB00, ATLISA 809M, ATLISA Tri 839MP, Tecnis Symfony ZXR00) and one monofocal IOL (Tecnis ZA9003), 6 months after cataract intervention.

Setting:

Ophthalmology Department of Santa Creu and Sant Pau Hospital, Barcelona, Spain.

Methods:

Patients were symmetrically and randomly implanted with AcrySof ReSTOR SV25T0 (n=19), ATLISA 809M (n=18), ATLISA Tri 839MP (n=19), Tecnis ZKB00 (n=20), Tecnis ZLB00 (n=20), Tecnis Symfony ZXR00 (n=20) and Tecnis ZA9003 (n=36). Six months after lens implantation, quality of life with the VF-14 questionnaire, subjective quality of vision (frequency of undesirable photic phenomena, such as shadows around letters or ghosting, halos and glare, and night driving difficulties) and spectacle independence at distance, intermediate and near vision were evaluated.

Results:

The Symfony obtained the best VF-14 scores, with significant differences with the SV25T0, ATLISA Tri 839MP and ATLISA 809M (p<0.05). The monofocal design offered best quality of vision in terms of glare and halos than all multifocal designs (all p<0.05, except for the SV25T0 with halos), with 77.8% and 94.4% of the patients reporting no glare and halos at all. No statistically significant differences were found amongst multifocal designs, although patients with ATLISA Tri 839MP and ATLISA 809M reported more glare. Spectacle independence at near was best with ATLISA Tri 839MP (77.8%), ATLISA 809M (76.5%) and ZLB00 (65.0%).

Conclusions:

Whereas multifocal lens designs, mainly those with high add powers, offer better spectacle independence at near than monofocal IOLs, all multifocal designs are associated with the perception of glare and halos. Notwithstanding the persistence of unwanted photic phenomena, patients implanted with multifocal lenses reported higher quality of life scores, mostly as these lens designs allow them to conduct daily life tasks without the need for spectacle correction, particularly at near.
