Alteration in protein and cytokine levels in the aqueous humor in eyes with implantation of iris-fixated anterior chamber phakic intraocular lens

Abstract Details

Purpose:

Reduction of endothelial cell density (ECD) after corneal transplantation has been recently reported to be associated with preoperative iris damage and inflammatory cytokine levels in the aqueous humor (AqH). On the other hand, there have been concerns that iris-fixated anterior chamber phakic intraocular lens (AC-pIOL) could damage endothelial cells postoperatively. To investigate the influential factors of postoperative ECD, we conducted a pilot study about the levels of protein and cytokines in the AqH of eyes with implantation of iris-fixated AC-pIOL.

Tokyo Dental College Ichikawa General Hospital, Chiba, Japan and Minamiaoyama Eye Clinic, Tokyo, Japan

Methods:

AqH samples of the study group were collected at the surgery of iris-fixated AC-pIOL removal combined with phacoemulsification and implantation of posterior IOL (N=6, mean age 54.5 ± 5.3 years). The mean ECD was 1767 ± 684 (range; from 533 to 2481) cells/mm2. AqH samples of the control group were collected at the surgery of age-related cataract (N=33, mean age 75.7 ± 7.7 years). Protein concentration in the AqH was determined by the DC protein assay using microplate reader. The levels of cytokines in the AqH were measured by multiplex beads immunoassay using Luminex.

Results:

The levels of AqH protein were significantly higher in eyes with anterior pIOL compared with the controls (0.93 ± 0.32, 0.31 ± 0.23 mg/mL, respectively, P = 0.0018). The levels of IL-1β were also significantly higher compared with those of the control group (9.09 ± 5.77, 3.14 ± 10.07 pg/mL, respectively, P = 0.046). The levels of IL-4, IP-10, TNF-α, and MIP-1β were significantly lower in the study group than those of the control group.

Conclusions:

This is the first report that showed the elevation of protein levels and alteration in cytokine levels in the AqH of eyes with iris-fixated AC-pIOL.
