Risk factors for graft failure after penetrating keratoplasty: a multivariate analysis of 895 procedures

Purpose:
Objective: To define principal risk factors for graft failure in patients who underwent penetrating keratoplasty (PK).

Methods:
Data obtained from 895 penetrating keratoplasties performed at the Centro de Oftalmología Barraquer between 2001 and 2006 were analyzed. Recipient and donor related factors as well as graft characteristics were assessed in a univariate analysis and with a Cox multivariate proportional hazard model.
Results:
Univariate analysis revealed significant survival differences for different diagnosis, recipient’s age, degree of corneal vascularization, re-grafting, preservation status of the donor tissue, time between donor’s death and transplant, graft diameter and combined surgery. Multivariate analysis confirmed diagnosis other than keratoconus and re-grafting were the strongest significant risk factors for graft failure. Ten-year survival estimate for keratoconus was 95% and less than 60% for other diagnoses. With respect to re-grafting, 10-year survival estimate was 81% for the first, 33% for the second and 16% for all subsequent grafts
Conclusions:
Preoperative corneal vascularization, recipient age, and time between donor’s death and transplant were significantly associated with graft failure in univariate but not in multivariate analysis. Primary diagnosis other than keratoconus and previous graft failures in the recipient are the most important risk factors of graft failure after a penetrating keratoplasty in proportional hazards multivariate analysis
