Abstract Details

Purpose:

Purpose:Demonstrate the advantage of using of Femtosecond for the creation of a flap over a previous one, already made with microkeratome, in order to avoid lifting it and cause secondary complications, such as descentration, dislocation and optical aberrations, among others.

Setting:

Centro Panamericano de Ojos 91 Avenida norte #541. Colonia Escalón. Clínica López Beltrán. San Salvador, El Salvador.

Methods:

Methods: Topographic evaluation by means of Pentacam, of patient operated of Lasik with microkeratome. The patient is a 38-year-old female patient who presents asthenopic symptoms related to a refractive error presented with a LASIK surgery 8 years ago, which was performed with a Carriazo microkeratome and LASIK AMARIS SCHWIND 500HZ ablation, was made a 90 um of flap thickness with LDV Z6 Femtosecond, 90 grades of bothe eye's hinges. No pathological personal history, best corrected visual acuity was 20/20 each eye with the following refraction: Right eye p-2.00x90 Left eye p-1,75x95. The rest of the ophthalmological examination was normal

Results:

Results: Satisfactory postoperative refractive correction. Refractive and topometric stability was obtained in relation to the creation of the flap with Femtosecond Z6, obtaining a satisfactory visual acuity without presenting corneal aberrations, displacement, distortion, dislocation of either of the two flaps and without alteration in the tear film, as it does not reflect damage to the ocular surface. The post op refractive results are: Right eye p-0,75x80, Left eye p-0,50x100.

Conclusions:

Conclusions: The use of the femtosecond, in this case Ziemer LDV 6, has an advantage over the microkeratome, not only in surgeries in virgin eyes, but also in those in which the first flap is contraindicated to be lifted up, either with femtosecond or microkeratome, avoiding aberrations, either by displacement of the flap or by instability thereof. Thus, the main advantage that the Femtosecond, in this case, is that we could perform thinner flaps over already created microkertome one, which those are thicker in the most cases, therefor we avoid cases of ectasia in the most of the patients.
