Cataract surgery using torsional without ultrasound power or nanolaser photofragmentation: corneal tissue trauma

Abstract Details

Purpose:

The aim of this study was to compare corneal tissue trauma induced by cataract surgery using torsional without ultrasound power or nanolaserphotofragmentation

Setting:

Department of Medical Science, Neuroscience and Sense Organs, Eye Clinic, University of Bari, Bari, Italy

Methods:

This is a single-center prospective randomized double-blind clinical study, enrolling 50 consecutive eyes of 50 patients who presented with a nuclear cataract of grade 3. Patients were randomly assigned in two groups:lens removal with either torsional without ultrasound power (Infinity Vision System, Alcon, TX) with a divide-and-conquer technique or nanolaserphotofragmentation (Infinity Vision System, Alcon, TX and Cetus Nanolaser System, A.R.C. Laser, Nuremberg, Germany) with a chop technique. The main outcome measures were corneal endothelial cell count and corneal thickness at the tunnel site and at the center of the cornea measured before and 1, and 30 days after surgery

Results:

To date, 18 (9 per group) patients were enrolled and completed their 30 day follow-up visit. The difference in best correct visual acuity between the two groups at 30-day was not significantly significant. Statistically significant differences between the two groups for the two main outcome parameter at the three time points were not observed (Mann-Whitney test).Group differences for the three time points was found for corneal thickness for the torsional group (p=0.045, Friedman Test) and post-hoc pairwise comparison of the torsional group indicated a statistically significant difference (p=0.049, Wilcoxon Signed Ranks Test) between pre-surgery and one day after surgery

Conclusions:

Although enrollment must be completed, initial results indicated that the increase in central corneal thickness following cataract surgery is greater when torsional technique is used for phacoemulsification compared to nanolaserphotofragmentation
