Abstract Details

Purpose:

To investigate the morphologic features of posterior capsule (PC)– intraocular lens (IOL) interaction following cataract surgery with primary posterior continuous circular capsulorhexis (PCCC) in pseudoexfoliation syndrome (PEX).

Setting:

The Acad. S.N. Fyodorov Eye Microsurgery Complex of the Health Ministry of Russia, Novosibirsk, Russia

Methods:

We examined 27 patients who had undergone uneventful phacoemulsification with capsular tension ring, posterior chamber IOL implantation and PCCC. Ophthalmic examinations were performed. Patients were followed up from 1 day to 1 month postsurgery. Optical coherence tomography (RTVue XR Avanti, «Optovue») of vitreolenticular interface were performed in all cases.

Results:

We have determined the morphological features of the vitreolenticular interface in PEX: destruction of the Wieger's ligament, distention of the Berger’s area, the anterior hyaloid destruction. The PC-IOL inadhesion is a prevalent capsule–IOL interaction on the first day after cataract. The IOL-PC distance and the retrolental space were bigger in the lower sector. The contact between optic and posterior capsule always progresses from the periphery to the center. The time needed for complete IOL-PC adherence was 3-8 days.

Conclusions:

It appeared that the morphological features of the vitreolenticular interface in PEX had a role in the IOL-PC interaction. The primary posterior capsulorhexis adhesion between the PC and the optical part of IOL occurs 3–8 days after surgery. The OCT is a good tool to observe the dynamics process of the capsular adhesion to different intraocular lenses.
