Purpose:
To analyze the possibilities of the influence of therapeutic soft contact lenses on the functional results of penetrating keratoplasty in patients with secondary endothelial epithelial dystrophy of cornea and pseudophakia.
Setting:
Academician S.N. Fedorov St.Petersburg Branch of the IR&TC "Eye microsurgery", St.Petersburg, Russia.
Methods:
A clinical trial was conducted in 54 patients who underwent penetrating corneal transplantation for secondary endothelial-epithelial dystrophy. Previously, patients underwent phacoemulsification of cataracts with the implantation of various models of intracapsular IOLs.
For patients of the main group, consisting of 32 patients, were applied silicone hydrogel therapeutic contact lenses "Air Optix Night & Day" (Ciba Vision) and "Pure Vision 2HD" (Bausch & Lomb) after the surgery.
Eye drops were buried on the upper segment of the contact lens. In the control group consisting of 26 patients with secondary endothelial epithelial dystrophy of the cornea, contact lenses were not used
Results:
The refractive spherical equivalent in both groups was 2.25±0.5 D and 3.75±0.5 D respectively. The mean visual acuity values in the main group were 0.42±0.018, in the control group 0.32±0.025. The cylindrical component of refraction in the main group averaged 3.25-0.5 D, in the control group it was 5.5±0.75 D (p <0.05). In the first group complete epithelization occurred in 3.5 ± 2.8 days, in the control group - after 5.7±2.3 days. During 17.5-7.5 days after the surgery 16 (29.6%) patients of the control group had photophobia, lacrimation and discomfort caused by sutures.
Conclusions:
The presented data evidences to the advisability of using therapeutic contact lenses with the possibility of improving the biological, optical and refractive results of penetrating keratoplasty. The use of therapeutic contact lenses increases CDVA by reducing the cylindrical component of refraction, contributes to accelerating the epithelization of the graft, increases the tightness of the anterior chamber and reduces postoperative discomfort.
