Purpose:
To study the microbial profile, antibiotic susceptibility pattern, risk factors, and outcome for cases of microbial keratitis presented to tertiary Cornea unit in Abu Dhabi over the a period of 3 years (between January 2011 and December 2015)
Setting:
A retrospective study conducted in a tertiary Cornea unit at Sheikh Khalifa Medical City in Abu Dhabi, UAE.
Methods:
All cases who had had corneal scraping for presumed infectious keratitis performed at Sheikh Khalifa Medical City, Abu Dhabi, between January 2011 and December 2015 were reviewed. Information concerning culture results, antibiotic susceptibility, risk factors and visual outcome were studied.
Results:
132 cases were identified aged 2 to 80 years. Culture was positive in 55 cases of which 40% grown
Pseudomonas aeruginosa, 40% Gram positive bacteria and 18% fungal infection.
All pseudomonas isolates were sensitive to tobramycin while most of Gram positive isolates were
sensitive to clindamycine. Therapeutic keratoplasty was needed in 60% of fungal keratitis.
Risk factors include chronic ocular surface disease in 35%, contact lens wearing 24% and trauma in 19% of cases. Vision at presentation ranged from Perception of light to 20/20 with significant improvement of median vision from 20/400 at presentation to 20/100) on discharge (P= 00)
Conclusions:
The results of the study represent the first UAE based report on local microbial profile and antibiotic susceptibility pattern for cases of microbial keratitis. The mixed nature of microbial pattern which includes contact lens related Pseudomonas keratitis and trauma related fungal keratitis reflects the cosmopolitan nature of the country. The antibiotic sensitivity profile support the use of combination of Tobramycin and Clindamycin as an initial empirical broad spectrum antibiotic therapy at presentation and while awaiting the results of culture and sensitivity. The study reveal poor prognosis for cases of fungal keratitis as more than half of them required therapeutic keratoplasty.
