Clinical and topographic outcomes of topography-guided surface ablation in highly aberrated corneas

Purpose:
To evaluate the changes in refraction, visual acuity and aberrations of highly aberrated corneas after topography-guided surface ablation.

Methods:
Retrospective study of patients with highly aberrated corneas due to post-herpetic and post-traumatic leucomas, decentered surface ablation or ablation with a small optical zone. Scheimpflug topography analysis (Wave Oculyzer), refraction, and a comprehensive ophthalmic examination were performed before and 6 months after surface ablation using excimer laser (Allegretto Wave Eye-Q 400Hz). Main variables were the best corrected visual acuity (BCVA), spherical and cylinder refraction, coma and total aberrations.
Results:
A total of 12 eyes of 8 patients were studied. Treatment indications were posttraumatic leucomas (3 cases), post-herpetic leucomas (2 cases), small optical zone ablation (3 cases), and decentered ablation (4 cases). Mean age was 31.7 (SD 5.8), 50% were male and 50% female. BCVA (logMAR) improved from 0.17 at baseline to 0.07 (p=0.01) six months postoperatively, spherical refraction decreased from -5.53 to -2.17 (p=0.001), cylinder from -4.16 to -1.75 (p=0.05), coma at 6mm from 2.14 to 1.33 (p=0.001) and total aberration at 6mm from 2.42 to 1.59 (p=0.01). There were no intraoperative or postoperative complications.
Conclusions:
In our sample, clinical and topographic analysis showed a significant improvement in all parameters studied, suggesting that topography-guided laser surface ablation may be a valuable and promising therapeutic modality for patients with highly aberrated corneas.
