LASIK difficulties in a case of epithelium basement dystrophy

Abstract Details

Purpose:

To present a case of epithelial basement membrane dystrophy with difficulties faced during LASIK surgery intraoperatively and postoperatively.

Setting:

Dr om parkash eye institute,amritsar,india

Methods:

25 year old patient was taken up for LASIK surgery. Preoperative evaluation was unremarkable. Intraoperatively, after applying suction at interface during femtosecond laser LASIK surgery in the right eye,epithelium became loose. The loose epithelium had to be fashioned. Bandage contact lens was applied. The other eye was planned for next visit. Postoperatively, he developed sands of Sahara which responded well to topical steroids. After a month, he presented with diminution of vision. Slit lamp evaluation revealed multiple areas of cells clumping in sub-epithelial area. This was validated using anterior segment optical coherence tomography.Epithelial debridement mechanically allowed healing of cornea completely.

Results:

Epithelium was healed after completely debriding it mechanically. Anatomical and functional outcome was satisfactory.

Conclusions:

Epithelial basement membrane dystrophy can present as a challenge during LASIK surgery.
