Abstract Details
Purpose:
To assess the accuracy of IOL calculation and visual outcome of post- small incision lenticule extraction (SMILE) cataract surgery with multifocal IOL implantation.
Setting:
Nethradhama Superspeciality Eye Hospital, Bangalore
Methods:
A 55 years-old male came with complaints of diminished vision in both eyes and past history of small-incision lenticule extraction for high myopia in both eyes performed 5 years ago. On examination, patient had grade 2 nuclear sclerosis in both eyes with normal posterior segment examination and was planned for cataract surgery with +19.5 D (RE)and +18,5 D(LE) with +3.25 D add MFIOLS in BE. Pre-operatively IOL master 700 and Pentacam were performed for IOL calculation and various IOL calculation formulae were considered. Post-operatively uncorrected distant, intermediate and near visual acuities, contrast sensitivity, stereopsis were assessed.
Results:
Biometry was performed using IOL master 700, however for keratometry equivalent keratometric (EKR) readings from Pentacam were considered. Using the Barrett’s universal 2 formula, IOL was calculated. Uncorrected distant visual acuity was 0.1 and 0 logMar in right and left eye, near visual acuity was N6 in both eyes, intermediate acuity was 0.1, 0, 0.2 logMar for 40, 60 and 80 cm. Stereopsis was 400 secs.Patient satisfaction was excellent for far, intermediate and near distances and he did not have any significant complaints of dysphotopsias,
Conclusions:
To the best of our knowledge this is the first ever case report of post-SMILE cataract surgery with multifocal IOL implantation. Post-operatively the patient had excellent visual outcome and complete spectacle independance. The Barrett’s universal 2 formula was accurate in calculating the IOL power.
