Purpose:
There are only limited numbers of reports on IOL power calculation in eyes with KAMRA and KAMRA is said to have little influence on it usually.
The purpose of this report is to demonstrate the unexpected influence of KAMRA insertion combined with LASIK on IOL power calculation, by describing one case with large postoperative refractive errors after cataract surgery.

Nishi Eye Hospital, Osaka, Japan
Methods:
A sixty-year old male consulted with decreased distant visual acuity of both eyes. The visual acuity of both eyes was 0.9 (n.c.) with a history of LASIK at other hospital. The left eye had KAMRA additionally.
Both segments were normal except nuclear cataract in the right eye (grade II) and in the left eye (grade IV). R1=9.12mm and R2=8.73mm in the right eye. R1=8.48mm and R2=8.13mm in the left eye.
As he wished, the left eye with more severe symptoms was firstly schedule to cataract surgery, followed by the right eye. Monofocal IOL was selected, aiming emmetropia, after well informed.
Results:
As the patient desired, the left eye with KAMRA combined with LASIK went on cataract surgery as planned, followed by the right eye two weeks later. Surgeries for both eyes were completed smoothly. The one day postoperative visual acuity of the left eye was 1.0 (1.0 x S +1.50D). The one week postoperatively, it changed to 0.4 (0.6 x S+1.25D:C-1.00DAx180°). The one month postoperative visual acuity turned out to be 0.1 (1.2 x S+7.00D:C-1.00DAx170).
On the other hand, the one month postoperative visual acuity of the right eye was 1.0 (1.2 x S-0.75D) and the refractive error was only 0.4D.
Conclusions:
This case indicated the possible risk of large refractive errors even with OKULIX, in case of IOL power calculation of eyes with a past history of KAMRA insertion combined with LASIK. Special caution is necessary when combined with LASIK additionally. As similar cases are expected to increase, further reports and investigation are essential.
