Purpose:
To report the outcomes in the first cohort implanted with a novel trifocal toric hydrophobic IOL with an intermediate focus at 60 cm.
Setting:
Private refractive surgery clinic: Retina Total Eye Care, Driebergen, affiliated with the Department of Ophthalmology Academic Medical Center, Amsterdam, the The Netherlands.
Methods:
Prospective clinical study involving the first cohort of 18 eyes (13 patients) implanted with the TFNT20-50 intraocular lens (Alcon, USA) during standard phacoemulsification cataract surgery or refractive lens exchange. All patients had regular corneal astigmatism preoperatively. Alignment was achieved using either the Nuijts-Mendez toric marker (ASICO®) or the Verion image guided system (Alcon®). Primary outcome measures are uncorrected and corrected distance acuity (UCDA and CDA), and uncorrected acuity at 40 and 60 cm. Secondary outcomes include spherical equivalent refraction (SE), residual astigmatism, change in preoperative corneal cylinder to post-operative refractive cylinder, and alignment of the toric component of the IOL.
Results:
Mean pre- and postoperative uncorrected distance visual acuity measured: 0.88 + 0.36 vs. 0.03 + 0.08 LogMAR, p<0,001. Mean pre- and postoperative corrected visual acuity measured 0.15 + 0.22 vs. -0.04 + 0.04 LogMAR, p<0,003 . Postoperative uncorrected visual acuity at 40 and 60 cm was 0.03 + 0.03 LogMAR, and 0.01 + 0.03 LogMAR, , respectively. Mean pre- and postoperative measured +0.16 + 4.77 D vs. -0.12 + 0.30 D, p<0,001. Mean postoperative refractive cylinder was reduced to -0.39 + 0.42 D. No complications were observed. Mean postoperative deviation of implanted IOLs from the intended axis was 1.89 degrees.
Conclusions:
Preliminary refractive and visual outcomes in the first cohort implanted with the TFNT20-50 toric IOL are promising. We report significant improvements in distance, intermediate, and near uncorrected vision with good correction of the cylindrical error and rotational stability without complications six-months postoperatively. Additional studies, including defocus curves, are needed to assess quality of vision with this novel lens.
