Prognosis of the sterile corneal infiltrate after corneal collagen cross-linking in keratoconus

Purpose:
To evaluate the prognosis of the sterile corneal infiltrates that occurred after corneal cross-linking (CXL) for progressive keratoconus.

Baskent University Faculty of Medicine, Ophthalmology Department, Adana, Turkey
Methods:
Eleven patients who had progressive keratoconus underwent corneal deepithelization and subsequently CXL. The accelerated CXL treatments were performed using a 30-minute riboflavin 0.1% soak and 10-minute ultraviolet-A (UVA) irradiance at a level of 9 mW/cm2. Total radiant exposure was 5.4 J/cm2. Three days after the procedure, the patients presented with corneal stromal infiltrates.
Results:
The study evaluated 170 eyes of 100 patients. Ten patients had peripheral corneal infiltrates and one patient had central corneal infiltrates. The peripheral corneal infiltrates were superficial and present at the 9.0-mm zone. Corneal culture was negative. Sterile infiltration was diagnosed and all affected patients were treated with topical corticosteroids, antibiotics and artificial tears. Complete resolution was achieved in all patients with sterile corneal infiltrate after four weeks of treatment. Sterile infiltrates were seen in 6.47 % of eyes.
Conclusions:
Good visual outcomes were obtained in all patients. Phototoxic effect on the corneal stroma after CXL may be possible etiology that triggers these infiltrates.
