Purpose:
To evaluate the corneal densitometry (CD) at different zones and anatomical layers, the corneal thickness spatial profile (CTSP) and the corneal volume (CV) distribution after uneventful Descemet membrane endothelial keratoplasty (DMEK)
Setting:
Department of Ophthalmology, Philipps University, Marburg, Germany
Methods:
One hundred eyes which underwent DMEK without intraoperative and postoperative complications were included (group 1). Scheimpflug CD of the optically relevant (0-2mm and 2-6mm) zones was evaluated. The CTSP was recorded at 3 concentric rings (2, 4, and 6mm) around the point of minimum corneal thickness (MCT). The CV was evaluated at 3 zones (3, 5 and 7mm) centred on corneal apex. Endothelial cell density (ECD) was also recorded. The measurements were performed preoperatively and at 3, 6, 12 and 24 months postoperatively. The 24-month results were compared to an age-matched control group of uncomplicated pseudophakic eyes (group 2; n=20).
Results:
Preoperative total CD at 0-2mm and 2-6mm was 33.9±10.7and 27.9±8.1GSU and decreased at 20.3±4.7 and 20.2±4.6GSU, respectively, at 24 months. The MCT was 623±70 preoperatively and 529±37μm at 24 months. The CTSP at 2,4&6mm was 543±34, 580±34 and 639±38μm, respectively, at 24 months. The CV at 3,5&7mm was 4.9±0.7, 13.8±1.9 and 28.4±3.7mm3, preoperatively, and 3.9±0.3, 11.6±0.7 and 25.0±1.5mm3 at 24 months. Twenty-four-month total CD at 0-2mm and corneal thickness at 6mm were significantly higher after uneventful DMEK compared to pseudophakic controls.
Conclusions:
Twenty four months after uneventful DMEK, CTSP of the central 4mm and CV at 3, 5 and 7mm zones were similar to uncomplicated pseudophakic eyes. However, central CD (0-2mm) and mid-peripheral corneal thickness (6 mm) were higher compared to uncomplicated pseudophakic eyes.
