Impact of residual astigmatism on performance of premium multifocal intraocular lenses

Abstract Details

Purpose:

To assess the effect of residual refractive astigmatism on the performance of different types of multifocal IOLs

Setting:

Optical Express, Glasgow, UK

Methods:

Three months postoperative refractive astigmatism and its impact on outcomes was compared between 3 groups of patients: Group A: 2,096 patients with an extended depth of focus IOL (Tecnis Symofny ZXR00), Group B: 741 patients with a diffractive multifocal IOL (Tecnis Multifocal ZKB00) and Group C: 255 patients with a rotationally asymmetric refractive IOL (Lentis Mplus LS-313, MF20) Refractive astigmatism, clinical and patient reported outcomes were compared between the three groups.

Results:

All three groups had comparable amount of residual astigmatism (Group A -0.57 ± 0.41 D, Group B -0.51 ± 0.38 D, Group C: -0.44 ± 0.40 D, p = 0.37). All three groups had comparable postoperative visual phenomena and difficulties with night driving. Three months postoperatively, 3.9% of patients in Group A, 2.6% in Group B and 3.1% in Group C were dissatisfied if their postoperative refractive cylinder was 0.50 D or less, whereas this percentage was 6.2%, 3.0% and 11.4% for groups A, B and C respectively if the residual astigmatism was 1.00 D or more.

Conclusions:

Extended depth of focus IOL and diffractive multifocal IOL patients showed greater level of tolerance with increasing postoperative refractive cylinder, whereas patients with a rotationally asymmetric refractive IOL were more likely to be dissatisfied with increasing levels of astigmatism.
