Three year observation of microdistortions in Bowman's layer after SMILE for myopia

Purpose:
To investigate the characteristics and changes of microdistortions in Bowman's layer for three years after small incision lenticule extraction (SMILE).
Setting:
Key Lab of Myopia, Ministry of Health, Department of Ophthalmology, Eye and ENT Hospital of Fudan University, Shanghai, China.
Methods:
In this non-randomized, prospective study, 50 eyes of 28 patients who received SMILE with spherical equivalent of -6.72±1.41 diopters were divided into two groups: the lenticule thickness of ≤135μm (Thin-Lens Group) and of >136μm (Thick-Lens Group). The routine postoperative examinations were performed preoperatively and 1 day, 1month and 3 years after SMILE. The microdistortions in Bowman's layer obtained by optical coherence to¬mography (OCT) images were analyzed digitally using software Illustrator and AutoCAD.
Results:
All patients completed follow-up with no complications. Three years after SMILE, 100% eyes achieved equal or better than preoperative corrected distance visual acuity. The total lengths of microdistortions at postoperative 1day, 1month and 3years were 348.12±264.57 DPI, 503.92±296.76 DPI and 189.22±177.74 DPI respectively (P<0.05). In Thick-Lens Group, the microdistortion lengths at each time point were: 405.52±260.80DPI, 571.79±289.77 DPI and 208.72±169.43 DPI respectively; while in Thin-Lens Group were: 256.81±259.08 DPI, 361.58±298.44 DPI and 169.72±190.49 DPI respectively. Between two groups, there was significant difference at 1day and 1month postoperatively (p<0.05).
Conclusions:
The microdistortions in Bowman's layer could last for at least 3 years after SMILE, but the total lengths decrease with the time. The thick lens' thickness is the major factor in the early occurrence of the microdistortions, but its effect decreased in long term.
