Abstract Details

Purpose:

To evaluate the ability of new geometrical parameters to characterize the keratoconus and to develop a detection model based on them.

Setting:

Vissum Corporation, Alicante, Spain. Technical University of Cartagena, Spain.

Methods:

This study comprised a total of 400 eyes: 201 keratoconus eyes (KC group), 78 eyes with early keratoconus (EKC group) and 121 healthy eyes (C group). In all cases, a complete eye exam was performed including an analysis of the corneal structure with the Sirius system (Costruzione Strumenti Oftalmici, CSO). The SolidWorks v2012 software was used to calculate in all cases the geometric parameters Aant, Apost, Aapexant, Aapexpost, Amctant, Amctpost, Dapexant, Dapexpost, Dmctant and Dmctpost of the anterior and posterior surfaces of the 3D virtual model of each cornea.

Results:

Significant differences among control and keratoconus group were found in Aapexant, Aapexpost, Amctant, Amctpost, Dapexpost, Dapexant (all p<0.001), Apost (p=0.012), and Dmctpost (p=0.031). Significant correlations in keratoconus group were found between Aant and Apost (r=0.881), Amctant and Amctpost (r=0.990), and Dmctant and Dmctpost (r=0.939, all p<0.001). A logistic regression analysis revealed that the detection of early keratoconus was related to Apost, Atot, Aapexant, Amctant, Amctpost, Dapexpost, Dmctant and Dmctpost (Hosmer-Lemeshow: p>0.05, R2 Nagelkerke: 0.919). The overall percentage of cases correctly classified by the model was 96.9%.

Conclusions:

The geometrical parameters defined in this study are useful for the diagnosis of keratoconus. The combination of Apost, Atot, Aapexant, Amctant, Amctpost, Dapexpost, Dmctant and Dmctpost with corneal aberrometric data provides a consistent detection model.
