Comparison of corneal endothelial cell loss induced by different cataract incisions in phacoemulsification using specular microscopy

Abstract Details

Purpose:

To analyse and quantitatively compare the Endothelial Cell loss in cataract surgery done by different incision types ( Scleral tunnel and clear cornea) and different incision sizes (1.8 mm and 2.8 mm), evaluating the endothelial cell density in the pre and post-operative period

Setting:

Shekar Super Speciality Eye Hospital, Bangalore

Methods:

120 Patients aged 50-70 years with senile cataract and without any corneal pathology underwent phacoemulsification with clear corneal incision(CCI) of 1.8 mm or 2.8 mm; or scleral tunnel incision(STI) of 1.8 or 2.8 mm. 30 patients were allotted to each group. An endothelial cell count (ECL), co-efficient of variation(CV), hexagonality and central corneal thickness(CCT) were measured preoperatively and 1 week & 3 months postoperatively. We considered >125 ECL as clinically significant change.

Results:

Average ECL in 1.8 mm STI and 2.8 STI group was less compared to 1.8 mm CCI and 2.8 mm CCI both at 1 week and 3 months postoperatively, but Difference was not statistically significant both at 1 week(p=0.403) and 3 month(p=0.558); but clinically significant(>125 ECL) in both CCI groups. At both follow up CV was increased in all groups and change was significantly more in 2.8 mm CCI group compared to other groups both at 1 week(p=0.078) and 3 month(p=0.041). There was no statistically significant change in CCT and Hexagonality in any group at both follow up.

Conclusions:

scleral tunnel incision is associated with less endothelial cell loss compared to clear corneal incision group. And in both Scleral tunnel and clear corneal group smaller incision associated with less endothelial cell changes. But the difference is not statistically significant for both incision sites and sizes.
