Comparison of three toric intraocular lenses for astigmatic correction in cataract surgery

Abstract Details

Purpose:

To compare the visual outcomes, astigmatic correction and rotational stability of the Acrysof (Alcon), Tecnis (Johnson & Johnson) and ATTorbi (Zeiss) toric intraocular lenses for cataract surgery.

Setting:

Lee Eye Centre Ipoh, Malaysia

Methods:

Retrospective study of consecutive patients who underwent cataract surgery with implantation of a toric intraocular lens (IOL). All eyes were divided into 3 groups – Acrysof, Tecnis and ATTorbi and IOL selection was done using the Barrett Toric Calculator. Postoperative astigmatic corrections were analysed using the Alpins Method based on 3 fundamental vectors: target induced astigmatism (TIA), surgically induced astigmatism (SIA) and difference vector (DV). Postoperative refraction and IOL axis of alignment was measured at 1 month.

Results:

Sixty-five eyes were included in this study – Acrysof(n=21), Tecnis(n=24) and ATTorbi (n=20). Postoperative mean logMAR uncorrected distance acuity was 0.11(Acrysof), 0.10(Tecnis) and 0.15(ATTorbi) and these were not significantly different(p=0.362). Mean IOL rotation was 3.19°(Acrysof), 4.58°(Tecnis) and 4.78°(ATTorbi) and were also not significantly different(p=0.443). Proportion of eyes with < 5° rotation were 90.5%(Acrysof), 70.8%(Tecnis) and 80%(ATTorbi). The Acrysof group had a mean SIA(1.75) greater than TIA(1.74) whereas Tecnis(SIA 1.51, TIA 1.52) and ATTorbi(SIA 1.62, TIA 1.73) showed the opposite. The mean DV was 0.44(Acrysof), 0.51(Tecnis) and 0.46(ATTorbi) and again not significantly different(p=0.566).

Conclusions:

All three intraocular lens platforms enabled good visual outcomes with predictable reduction in astigmatism. There was overall good rotational stability for all 3 platforms but the Acrysof was on average, the least likely to rotate. The Acrysof also tended to overcorrect whereas the Tecnis and ATTorbi tended to undercorrect the astigmatism.
