Abstract Details

Purpose:

To evaluate the refractive & functional results, defocus curve, disphotopic phenomena and patient satisfaction and after binocular implantation of an enhanced depth of focus (EDOF) intraocular lens (IOL) compared to a Mix & Match approach with the same IOL in the distance dominant eye and a trifocal IOL in the near dominant eye.

Methods:

In a prospective study cataract patients received either binocular an EDOF IOL (AT LARA 829, Carl Zeiss Meditech, Germany) targeted emmetropia (EDOF group) or the same IOL in their distance dominant eye and a trifocal IOL (AT LISA 839, Carl Zeiss Meditech, Germany) in their near dominant eye (Mix&Match group). Pre- and postoperative, monocular and binocular functional results (CDVA, UDVA, DCIVA [90cm, 80cm, 60cm], DCNVA, UNVA, Defocus curve; [logMAR]) refractive outcome, Halo and Glare Simulator and patient satisfaction score were evaluated.

Results:

91% in the EDOF and 77% in the Mix&Match group had a postoperative SE in between 0.75D. Mean binocular UDVA, CDVA, DCIVA (90cm, 80cm and 60CM) DCNVA in the EDOF and the Mix&Match group were -0.03, -0.05, -0.02, 0.03, 0.05, 0.33 (EDOF group) and -0.05, 0.00, -0.05, 0.13, -0.08, 0.02 (Mix&Match group), respectively. Halo & Glare analysis showed 85,8% of EDOF patients reporting none to mild and 100% of Mix & Match patients reporting mild to moderate strength. The Mix&Match group questionnaire evaluation showed a higher degree of spectacle independence in near distance compared to the EDOF group.

Conclusions:

Both groups show a high degree of patient satisfaction. The Mix & Match approach clearly shows a higher degree of spectacle independence in near, while still resulting in a slightly higher perception of disphotopic phenomena compared to the EDOF group. Careful patient selection is the key for individualized presbyopia treatment.
