Fixation-stability and visual outcomes of the rhexis-fixated FEMTIS-IOL after automated femtosecond laser-assisted capsulotomy: interim results of the FEMTIS multicentre study

Abstract Details

Purpose:

In this study, we evaluated the decentration-, rotational- and tilt behavior as well as visual acuity, subjective refraction and secondary cataract rate of an intraocular lens that had been enclaved into the capsulorrhexis during cataract surgery.

Setting:

University Eye Clinic of Heidelberg and the FEMTIS multicentre study group

Methods:

294 eyes of 156 patients with cataract and expected postoperative corneal astigmatism of under -1.00 D were included. In 282 eyes, a monofocal IOL with 4 additional haptics for rhexis-fixation (FEMTIS, FB-313G; OSD GmbH, Berlin, Germany) was implanted. The uncorrected and best-corrected distance visual acuity, secondary cataract rate and a questionnaire for surgeons were evaluated preoperatively, intraoperatively and 1-7 days, 6-8 weeks, 6, and 12 months postoperatively. For the assessment of the centration- and positioning behavior, images were obtained via microscope, slitlamp, and Scheimpflug imaging and analyzed using a dedicated analysis program by Heidelberg Institute of Applied Mathematics.

Results:

The mean best-corrected distance visual acuity in logMAR improved from 0.25 (± 0.18) preoperatively to 0.00 (±0.09) 6-8 weeks, 0.00 (±0.11) 6 months and -0.04 (±0.10) 12 months postoperatively. The average IOL-rotation was 2.01° (±1.54°) 1-7 days, 1.17° (±0.86°) 6-8 weeks, 0.89° (±0.68°) 6 months and 0.78° (±0.81°) 12 months postoperatively. The mean IOL decentration was 0.10mm (±0.10 mm) 1-7 days, 0.07mm (±0.07mm) 6-8 weeks, 0.08mm (±0.06mm) 6 months and 0.07mm (±0.10mm) 12 months postoperatively. The current YAG-laser treatment rate is 0.7% 6 months postoperatively and 1.1% 12 months postoperatively.

Conclusions:

The rhexis-fixated FEMTIS-IOL shows very good results regarding the uncorrected and best-corrected distance visual acuity and excellent rotational as well as centration behavior. Compared to historical data, the FEMTIS-IOL was superior to the conventional in-the-bag implanted IOLs.
