Abstract Details

Purpose:

The implantation of the IC-8 small aperture IOL for patients with irregular corneal astigmatism undergoing cataract surgery might improve visual acuity and visual quality due to a pinhole effect. Even more these patients might benefit from an enhanced depth of focus, which could result in spectacle independence.

Setting:

Department of Ophthalmology Ludwig-Maximilians-University, Munich, Germany.

Methods:

This prospective, non-randomised, single-centre, interventional case series enrolled patients with irregular corneal astigmatism (mild or moderate non-progressive keratoconus, pellucid marginal degeneration, s/p corneal trauma, radial keratotomy and keratoplasty) and cataract. Further inclusion criteria were CDVA ≤ 0.7 Snellen, age above 45 years and intolerance in wearing rigid contact lenses. Preoperative and postoperative UDVA, UIVA, UNVA, CDVA, CIVA, CNVA monocular and binocular were obtained. Defocus curve was also assessed. Quality of life questionnaire (NEI-RQL-42TM) was used to evaluate the subjective improvement. The fellow eye with a clear lens or monofocal IOL served as control group.

Results:

Surgery was uneventful in all cases. No postoperative complications were reported. Preliminary results showed an improvement in uncorrected distance, intermediate and near visual acuity of 0.66 LogMAR, 0.62 LogMAR and 0.56 LogMAR, respectively. Defocus curve showed an enhancement of depth of focus in all patients. According to the analysis of quality of life questionnaire subjective complaints, such as dysphotopsia and photophobia showed a significant improvement.

Conclusions:

The implantation of the Small-Aperture IC-8 IOL in patients with irregular corneal astigmatism and cataract is a safe and effective surgical procedure. This is an ongoing study and its preliminary results show a significant visual restoration with improvement of the defocus curve and optical phenomena. Final results including three month refractive and visual outcomes as well as patient selection regarding the underlying corneal pathology, will be presented at the conference.
