Effect of IOP-lowering surgeries on subfoveal choroidal thickness: a comparative study

Abstract Details

Purpose:

The aim of our study is to compare the effect of trabeculectomy and deep sclerectomy on the choroidal thickness with SD-OCT after IOP reduction in patients with POAG.

Setting:

Cairo university hospitals

Methods:

Prospective interventional comparative non randomized study. Forty patients indicated for surgical intervention were recruited from the Glaucoma Subspecialty Clinics in Kasr Al-Aini University Hospital during the period from May 2015 to October 2016. Patients fulfilling the inclusion criteria were informed by the study purpose then they were divided into two groups: Group A: 20 patients each subjected to trabculectomy. Group B: 20 patients each subjected to deep sclerectomy surgery. Subfoveal CT (SFCT) was measured using SD-OCT preoperatively, 3rd day and 1 month postoperatively. change in IOP was correlated with change in the SFCT.

Results:

3rd day there was a significant increase in the SFCT of 42.8 um in the trabeculectomy group (p value=0.0000399). significant increase in the SFCT after one month of 28.5 um ,(p value = 0.0287). deep sclerectomy group, on the third day, a significant increase in the SFCT of 38.45 um (p value=0.0001). one month the increase in SFCT in the deep sclerectomy group was 13.25 um (p value = 0.19). The correlation between the IOP reduction and the increase in the SFCT was statistically significant in trabeculetomy group on third day & one month (p value=0.0016 and 0.0432).

Conclusions:

The large IOP reduction after both trabeculectomy and deep sclerectomy caused an increase in the SFCT which correlated with greater IOP reduction with no statsitical difference between both surgeries. The increase persisted for at least one month postoperatively in the trabeculectomy group. However, after one month postoperatively the increase in SFCT in the deep sclerectomy group became non significant. The choroidal thickening could have an inhibitory effect on glaucoma progression..
