IOL design and material influence in Nd:YAG laser rates for a large series of MICS IOL implantations

Abstract Details

Purpose:

To study the cumulative neodymium: YAG (Nd:Yag) laser rate (capsulotomy for posterior capsular opacification) after 6269 implantations during cataract surgery of 5 different hydrophilic acrylic MICS lenses and 1 hydrophobic and evaluate influence of material and design.

Setting:

Centre Ophtalmologique IRIDIS, Albi, France.

Methods:

This retrospective study comprises all patients who were implanted between 2006 and the end of 2017 with one of the after 6 micro-incision IOLs: Akreos MICS and INCISE (Bausch and Lomb), CT Asphina 509 M/MP (Carl Zeiss Meditec), MicroSlim, Micro AY and MicroPure (PhysIOL). The postoperative follow-up ranges from 1 to 144 months. The cumulative Nd: Yag laser frequency rates in all groups were calculated, and the cumulative incidence rates were defined by propensity score, Weighted Cox regression, Tuckey test, Kaplan-Meier survival analysis and Mann-Withney test.

Results:

The 2 years survival percentage was 100% for the INCISE (95% CI: 100-100); 74,11% for the Akreos MICS (95% CI: 68-79); 92,29% for the CT Asphina (95% CI: 90-94); 89,35% for the MicroSlim (95% CI: 86-92); 88,20% for the Micro AY (95% CI: 87-90) and 82,74% for the MicroPure (95% CI: 68-91). The 7 years survival percentage was 15,24% for the Akreos MICS (95% CI: 10-22); 30,12% for the CT Asphina (95% CI: 24-36); 67.59% for the MicroSlim (95% CI: 62-73); 50,02% for the Micro AY (95% CI: 47-53). No data available for the others IOL's too short follow up.

Conclusions:

At two years the INCISE has the best result with no ND:Yag laser capsulotomy and despite his hydrophobic material the MicroPure doesn’t seem to decrease ND:Yag. The CT Asphina plate haptic design shows a low Nd:Yag rate at 3 years but increases dramatically after with a tendency to have a higher rate at 7 years. Akreos Mics have the lower survival rate. MicroSlim gives the best Nd: Yag survival rate perhaps due to turning operation. The INCISE gives a lower PCO rate but with some side effects. All impacts of the IOL design will be discuss.
